Prognostic value of exercise stress echocardiography in patients with known coronary artery disease

Background Exercise stress echocardiography (EE) is useful for diagnosis and risk stratification in patients (pts) with suspected coronary artery disease (CAD). Pts known CAD carry a high events. Our aim was to assess the utility EE predicting outcomes this population. Methods Single center retrospective study consecutive pts who performed an between 2018 2019. The primary endpoint composite admission acute syndrome (ACS), revascularization cardiovascular death during follow-up. Results A total 76 were included (mean age 59±9 years; 87% male). Fifty-nine (78%) had history ACS, 51 (67%) percutaneous intervention (PCI) 14 (18%) bypass graft. majority 2 or more vessel (42 pts; 55%). main reason performing new onset chest pain (38 50%) followed by functional assessment stenoses after incomplete (29 38%). preserved left ventricular ejection fraction (67; 88%). exam under beta-blocker effect 35 (46%) pts. results positive myocardial ischemia 7 (9%) pts, negative 37 (49%) inconclusive 32 (42%) Mean exercise time 8±3 minutes mean METs achieved 9.4±2.6. ST-segment depression fulfilling electrocardiographic criteria occurred 17 (22%) Eleven (14%) complained exam. During median follow up 22 months (IQR 15-26), 9 (admission ACS 5 chronic 4 pts). No occurred. predictive value 57.1% (NPV) 97.3%. In survival analysis, predictors lower time, achieved, untreated significant lesions (figure).
